Description

This is a jar that contains a testis and its spermatic cord. The testis is of normal size. It is black in colour indicating gangrenous changes.

Some possible questions

Q. Is it truly torsion of the testis?

A. In fact it is torsion of the spermatic cord but the term "torsion of the testis" is commonly used.

 

Q. What are the predisposing factors?

 

Q. What are the clinical features?

 

Q. What is the differential diagnosis?

 

Q. How to diagnose it?

 

Q. Even after investigations you are not sure whether this is torsion of testis or acute epididymo-orchitis. What would u do?

A. It is safer to do emergency surgery to save the testis if it is still viable. Even if it turns out to be epididymo-orchitis there is no harm done.

 

Q. What would you do if the testis is viable?

A. Untwist and fix the testis (orchidopexy) to prevent recurrence.

 

Q. What would you do if the testis is gangrenous, like in this case?

A. Orchidectomy.

 

Q. In either case, how would you deal with the contralateral testis?

A. Orchidopexy in the same session because the predisposing factor to torsion is commonly bilateral.

 

Q. Can this be an operative specimen?

A. Yes, because this I a gangrenous testis whose treatment is orchidectomy.
