Specimen:

Both kidneys, ureters and bladder.

 

Gross Pathology :

1.   The bladder wall shows a fungating polypoid greyish white mass with areas of hemorrhage and necrosis. The tumor is projecting in the lumen and infiltrating the bladder wall.

2.   The lymph nodes at the bladder base are enlarged and infiltrated.

3.   The kidneys show discrete small metastatic nodules.

4.   The right ureter is dilated, the left ureter is slightly dilated. The pelvi- calyceal systems of both kidneys are mildly dilated.

 

Diagnosis :

1.  Polypoidal carcinoma of the bladder.

2.  Metastases involving both kidneys and regional lymph nodes.

3.  Bilateral hydroureter and hydronephrosis.
