Specimen:
Part of the left ventricle, aortic valve and root of the aorta.
Gross Pathology:
1.  The aortic cusps are yellowish.
2.  One of the aortic cusps shows irregular perforation.

Diagnosis:
Acute infective endocarditis of the aortic valve with cusp perforation.
